As neurons die, so do the brain&#8217;s executive functions\u2014the systems responsible for judgment, sequencing, impulse control, and recognizing appropriate behavior.<\/p>\n<p>Eventually, the brain begins relying more on primitive survival instincts.<\/p>\n<h3>The focus becomes:<\/h3>\n<p>Eat.<br \/>Sleep.<br \/>Eliminate.<br \/>Keep moving.<\/p>\n<p>The brain isn&#8217;t trying to embarrass anyone.<\/p>\n<p>It&#8217;s simply doing the best it can with the pathways that remain.<\/p>\n<p>The Biology Hasn&#8217;t Changed\u2014Our Perspective Has<\/p>\n<h3>Here&#8217;s something I&#8217;ve often thought about.<\/h3>\n<p>Baby messes? People laugh. Some take pictures and tell stories for years.<\/p>\n<p>Elder care messes?<\/p>\n<p>Most people freeze, panic, reach for disinfectant, and wonder if they&#8217;re failing.<\/p>\n<p>Yet biologically&#8230;<\/p>\n<p><strong>It&#8217;s the same digestive system.<\/strong><\/p>\n<p>The same human body.<\/p>\n<p>The same basic functions.<\/p>\n<p>The difference isn&#8217;t biology.<\/p>\n<p>It&#8217;s culture.<\/p>\n<h3>Somewhere along the way, we&#8217;ve learned to celebrate dependency in children while feeling uncomfortable with dependency in older adults.<\/h3>\n<p>But dementia doesn&#8217;t follow our social rules.<\/p>\n<p>It gradually rewinds the brain.<\/p>\n<p>Sensory experiences become more important than logic.<\/p>\n<p>Emotions become stronger than reasoning.<\/p>\n<p>Safety becomes more important than social expectations.<\/p>\n<h3>Why Caregivers Feel So Unprepared<\/h3>\n<p>When caregivers tell me, &#8220;I wasn&#8217;t built for this.&#8221; I don&#8217;t believe that&#8217;s really what they&#8217;re saying.\u00a0What they&#8217;re actually saying is:<br \/>&#8220;No one prepared me for this.&#8221;<\/p>\n<p>Most of us never learned how to care for someone whose brain is changing.<\/p>\n<p>No one taught us what happens when executive function disappears.<\/p>\n<p>No one explained why bathing becomes frightening, why loved ones wander, or why they may no longer recognize a toilet\u2014or know what to do once they get there.<\/p>\n<p>Instead, caregivers are handed an impossible job and expected to figure it out through trial and error.<\/p>\n<p>No wonder stress levels soar.<\/p>\n<p>According to the Alzheimer&#8217;s Association, millions of family caregivers provide billions of hours of unpaid care each year, and many report significant emotional, physical, and financial strain.
